Integrated Care Pathways (ICP)
Welcome to the dedicated page focused on supporting software vendors who may be interested in supporting Integrated Care Pathways (ICP).
What is it?
Integrated Care Pathways (ICP) is the result of our four-year Escalated Care Pathways (ECP) pilot that’s coming to an end. Integrated Care Pathways has been designed for patients needing more integrated, specialised, and coordinated treatment.
Broad introductory material to ICP is available here:
Integrated Care Pathways: New service coming soon (acc.co.nz)
Escalated Care Pathways (ECP) pilot continues to show positive benefits (acc.co.nz)
What are the benefits?
We found there was an opportunity for us to be more integrated and innovative with the sector, increase equity to services, and improve outcomes for patients.
This is an innovative way of managing people with injuries that require multiple rehabilitation services. The aim is to improve outcomes for patients with similar injury types and increase the quality and efficiency of their recovery.
Dates of importance
We’re on track for its upcoming operational go live in March 2024.
What is the status/current status?
We are continuing to work with Practice Management System Software Vendors wishing to support our Integrated Care Pathway service.
